CONFIDENTIALITY AND DISCLOSURE WAIVER
As a drop-in client training in/with Pinnacle Performance Ltd. (Pinnacle) I agree to the following clauses;
I understand that Pinnacle training programmes are owned and distributed by Pinnacle;
I agree to not making copies of, take photographs or videos of, or otherwise distributing Pinnacle training programmes to any other party;
I understand that Pinnacle hold final decisions regarding distribution or copying related issues;
I agree to the right to password protect my individual training programme and disclose the password to Pinnacle staff;
I agree to the use of photos and videos that I might be in during training at Pinnacle.
As a drop-in client training in/with Pinnacle I agree to the following medical and health and safety disclosure;
I understand that I am undertaking physical exercise which at times may induce an increased physical response. I agree to train within the guidelines and support of the Pinnacle coaches at my own risk;
I understand that I have the right to withdraw from any exercise or session whilst training;
I agree to the consideration of the training facility, myself, Pinnacle coaches and other members during my training sessions, both coach led or individual;
I agree to report any illness, injury or concerns that Pinn^cle should be aware of ahead of prescribing any training regime or during a training session;
I agree to disclose any previous medical history, current medical conditions or other identified issues that may be of relevance to a physical development programme;
I agree to participate in all activity at my own risk and responsibility;
I agree to release, indemnify and hold Pinnacle, its employees and agents, harmless from and against all and any actions or claims which I, or any party on my behalf, may bring arising out of any injury, loss, damage or death caused to me or my property in any way whatsoever.
